Help improve services for older men in the East Midlands 8th July 2008
Age Concern in the East Midlands is carrying out a survey of Older Men in the region to try to discover what we can do to improve the services we provide.
If you are aged 60 or over, we would be very pleased if you could find the time to complete the questionnaire as we are very keen to get your views on the questions. It does not matter whether or not you have used Age Concern services as your views will be equally welcome if you have not.
The questionnaire is available in in Microsoft Word format and Adobe Acrobat pdf format.
Completed questionnaires can be returned by email, or posted to

All returned questionnaires will be treated in confidence as we will not know who returned them. We only ask for the first part of your postcode so we know roughly which part of the region you come from.
